OakTown 4-H Member Supplemental Enrollment Form:
2026 Oak Town 4-H Member Supplemental Enrollment Form
This will include Expectations, Project Sign Up and Additional Enrollment Links
This form serves many purposes.
1)To make sure all families are aware of what the expectations are for a positive 4-H experience, we ask all families to complete our supplemental form. We require that you sign that you have read and are familiar with what makes your family's 4-H experience meaningful.
2) We ask (but do not require) all families to help with some tasks that make our club function more efficiently. These are quite varied in time commitment and dates they are needed.
3) Project Signups are handled with this survey. You sign up for a "Group Determined" project when you enroll in ZSuites, however, your Projects are assigned by completing this form. Some Projects have age restrictions and limited room for project members. Your enrollment in specific projects is done through t this form.
4) Information on additional "County-Wide" Projects, often focused on older youth members.
5) A way to copy and upload the members Health Form which is NOT readily available to Project and Club Leaders. IF you do not upload their Treatment Authorization and Health History here, the Project Leaders will not know if your member has any special needs or allergies.
5) Payment link if you have not already taken care of that.
